Salary – (Grade 7): £28,356 - £35,025 (£37,098 unconsolidated maximum through performance-related progression)

Sutton’s Hospital Pathway Team in Adult Social Care supports Sutton residents through the whole process surrounding hospital admission, enabling individuals to regain independence.  As part of the Sutton Health and Care alliance, Adult Social Care has strong multi-disciplinary partnership working across Health and Social Care, and our inhouse reablement team which is part of this alliance.  The Hospital Pathway Team is a multidisciplinary team which works with individuals to prevent hospital admission, to support hospital discharge, and throughout the whole period of reablement following discharge from hospital.

Assessment officers in the Hospital Pathway team complete assessments alongside the reablement team to achieve the best outcomes for individuals using a strengths-based approach, regularly completing eligibility assessments at the end of a period of reablement under the care act.  

Assessment Officers work closely with Social Workers, Occupational Therapists, Physiotherapists, and Admiral Nurses to identify strengths to enable people to live more independently through the use of community resources and commissioned services.

As part of this role, Assessment Officers will also be required to identify the need for Mental Capacity Assessments and Best Interest Decisions, NHS Continuing Healthcare assessments, applications to Court of Protection regarding deputyship for finances  and work with the social workers undertaking these. Assessment Officers may also support individuals to address issues relating to the habitability of their property.

As part of providing a holistic assessment, the Assessment Officers within the team can also assess and provide basic equipment to promote independence.  The accredited Trusted Assessor Training is provided to facilitate this.

About You:

We welcome applications from individuals who have experience of working in Adult Social Care, assessment writing and being able to deal with crisis situations that require intervention. You will be support planning with individuals and supporting those with dementia so experience in this area would be desirable.
